Take this as it's meant, do not be offended, but have you tried seeing a good therapist? They can help people become unstuck and move forward. Sounds like something is causing you to relapse and without deep conversations with a professional, it will be tough to uncover the issues here.
You can't voluntarily decide not to think of something. It's like telling someone "don't think about a pink elephant." What do you think they'll think of first? If you forbid yourself from certain thoughts, I guarantee it - you won't get past 3 days. Take it easy on yourself bro. If you don't give your fantasizing any oxygen (by not masturbating or watching porn), you'll eventually see the fantasies become more tame and fade away to an extent (they won't ever fully go away, though). It's just normal, otherwise our species would have died out since long because of having no offspring. Supressing the pink-elephant-thoughts won't work. From my point of view the mission for us is to find the right means to get by these toughts without masturbating, as hhh999 already wrote. To find these means you'll have to tackle the pink elephant directly.
