Welcome aboard @YellowDog - I like the username! Remember that hope is not a strategy. You need a solid plan. Take a look at this list of tools I use (and will update it with a few more implemented in the meantime!) The biggest is getting the involvement of others. Porn and masturbation thrive in aloneness, in loneliness, in isolation. So you've got to get it all out in the light. Get used to talking about porn and masturbation with others; get an accountabilty partner; get therapy; reach out to your community (church or SAA or something).
