I have a lot of disturbing thoughts that are related to watching porn, disgusting things that I watched or that the mind creates, it pops up my thoughts involuntarily like thoughts about deviations I'm afraid it's relapse because I don't fantasize it's thoughts that pop up involuntarily

It's not a relapse because it worries me, maybe it's like a fantasy

Don't worry. First you have to get rid of these thoughts. Stop thinking them. Just like stop button. It's not easy but we can try 100 times and succeed once. Engage in other activities and forget them. Write them down. You have to get at distance to them. So that they don't interrupt your daily life.

I know they aren't real, what scares me is that maybe it counts as fantasy again

They have power of course. It's not easy. They are deep rooted issues and may go back to your childhood. You have to check your relationship with your father and mother. Fantasies go with deep childhood issues. Mostly stem from parental contexts.
