Constant urge to orgasm and dry rough skin on penis glans

I’m constantly on the verge of orgasm even though I’m not turned on or having zero sexual thoughts. I'm only 19 and i already have dry and rough skin structure on my penis which feels and looks not so nice. I think i have lost some feeling on my penis glans skins. It feels weird when i touch it and not pleasant at all. My erection only lasts for 10 to 20 seconds before i ejaculate. Also, i seem to release sperms while urinating. There is no pain while i urinate. My groin area feels very warm. I dont know whats happening and dont know what to do. Im very scared and stressed. Is this going to make me infertile? Am i at a risk for prostrate problems? I never had sex and I used to masturbate excessively. I decided to quit on December 31st but relapsed after a week which was yesterday.That's when all these problems started to show up. Please Help! I'm currently a freshman in college and i dont want this to affect my studies. Please help a brother out!
